The following registration forms must be completed for each dog using Ciao Bow Wow’s services:
In addition, we require a letter from your veterinarian confirming that your dog is current in his/her vaccinations for rabies, distemper, bortadella and parvo.
Additional Requirements for Daycare:
The health and safety of every dog is our first priority. Accordingly, we require the following for daycare services:
- Each dog must have a temperament evaluation to determine his/her social skills and the best playgroup placement. The purpose of the evaluation is also to determine if daycare is right for your dog. While most dogs love daycare, it is not the best choice for every dog. Our evaluation ensures that our daycare services are the best choice for your dog.
- Each dog must be in good health
- Each dog must be friendly and non-aggressive
- Each dog must be neutered or spayed by the age of 7 months
There is a fee of $50 for daycare evaluation, which includes 3 hours of trial daycare on the day of the evaluation.
